Total T4 blood test: normal range explained
Also printed as: T4, T4 Total, Thyroxine, Total Thyroxine
Total T4 measures all the thyroxine in your blood, both the hormone bound to carrier proteins and the small free share. Testing.com prints 5.1–11.9 µg/dL for women over 20 and 4.9–10.5 µg/dL for men, together 4.9–11.9 µg/dL; Indian labs often print a wider range, but the range printed on your own report is the one that applies.

What is the normal range for total T4?
| Printed for | Range |
|---|---|
| Adults over 20, women and men together, as Testing.com prints them | 4.9–11.9µg/dL |
| Women over 20, as Testing.com prints it | 5.1–11.9µg/dL |
| Men over 20, as Testing.com prints it | 4.9–10.5µg/dL |
Reference ranges vary between laboratories. Always use the range printed on your own report. Why ranges differ between labs

What does total T4 measure?
Total T4 measures thyroxine (T4), the main hormone the thyroid gland makes, counting the hormone attached to proteins and the free hormone together. Bound T4 stays in the bloodstream as a backup supply; only the free share enters tissues and acts.
Because it includes the bound share, total T4 rises and falls when carrier proteins change, even if the active hormone does not. That is why MedlinePlus says free T4 is used more often, and NIDDK says many healthcare professionals prefer to measure free T4.

Why do doctors order the total T4 test?
Doctors order T4 to help diagnose and follow an underactive or overactive thyroid, to look into a goitre, thyroid nodules or pituitary problems, and to check people on thyroid hormone medicine. Testing.com lists weight change, tiredness and a racing heart among the symptoms that prompt the test.
A total T4 is usually read with TSH, which NIDDK describes as the first check, and sometimes with total T3. An abnormal TSH is followed by at least one other test, and MedlinePlus lists an abnormal TSH among the reasons for a T4 test.

What can change your total T4 result?
Total T4 is especially sensitive to anything that changes the proteins that carry thyroid hormone.
- Pregnancy, oestrogen therapy and birth control pills raise thyroid-binding proteins, which pushes total T4 up without more active hormone.
- Severe illness, major surgery and prolonged fasting can lower T4 even when the thyroid is healthy; corticosteroids can lower it too.
- Biotin supplements can interfere with the assay; Testing.com says to stop them at least 72 hours before the draw, so ask your doctor.
- Thyroid hormone tablets taken just before the sample can raise the reading, so the blood is usually drawn before that day's tablet.
- Too much iodine, from some medicines or seaweed, is linked to high T4, and too much or too little iodine in the diet to low T4.
- Inherited changes in the binding protein, and liver or kidney disease, can also affect the result.

How do you read total T4 on your lab report?
Reports print total T4 as T4, Total T4 or Thyroxine, in µg/dL or nmol/L, usually beside T3 and TSH. Some labs print a free T4 index (FTI), calculated from total T4, instead of a measured free T4.
Read the value against the range on your own report; Testing.com says ranges are approximate and vary by lab and assay, and it prints separate ranges for women, men and children. A result outside the range is read with TSH, your symptoms and earlier reports, and MedlinePlus says it does not always mean a condition needing treatment.
Amara reads total T4 with the range printed beside it on each report, flags a value outside that range, and shows what changed since your last report. Try it with two sample reports
A sample Amara row for Total T4: 14.6 µg/dL on 3 Dec 2025 at Lab A (sample), then 9.8 µg/dL on 15 Jul 2026 at Lab B (sample), each with its lab's printed range.

Questions people ask about total T4
Can pregnancy change total T4?
Pregnancy raises the proteins that carry thyroid hormone, so total T4 reads higher even when the active hormone is unchanged. NIDDK says thyroid hormone levels are higher during pregnancy and with oral contraceptives, and Testing.com describes the same effect. NIDDK says many healthcare professionals prefer free T4 for that reason.
What is the difference between total T4 and free T4?
Total T4 counts thyroxine bound to proteins plus the free hormone; free T4 counts only the unbound share that can enter your tissues. MedlinePlus says free T4 is considered more accurate and is used more often, because total T4 changes whenever binding proteins change, as in pregnancy or with oestrogen.
Do I need to fast for a total T4 test?
Fasting is not required for a total T4 test; Testing.com says to eat and drink normally. MedlinePlus adds that fasting for several hours may be needed only when other tests ordered on the same sample call for it, so follow the instructions the lab gives you for the whole form.
What does a high total T4 on my report mean?
High total T4 means the value sits above the range your lab printed. MedlinePlus lists an overactive thyroid, thyroiditis, too much iodine and excess thyroid medicine among the reasons, and pregnancy or oestrogen can raise total T4 through binding proteins alone. Your doctor reads it with TSH and your history.
