Free T3 blood test: normal range explained

Also printed as: FT3, Free Triiodothyronine, T3 Free

Free T3 measures the small, unbound share of triiodothyronine, the active thyroid hormone that your cells can use, rather than the larger amount carried on proteins. The adult range commonly printed on Indian reports is 2.3–4.2 pg/mL, as Testing.com prints, but the range printed on your own report is the one that applies, and it is read with TSH.

What does free T3 measure?

T3, triiodothyronine, is one of the two main thyroid hormones. Most of it travels bound to proteins, which keeps it from entering tissues; the free T3 is the unbound part that can act on cells. A free T3 test measures only that free part, while a total T3 test measures bound and free together.

T3 is less than 10% of the thyroid hormone in the blood but about four times more powerful than T4, so it drives much of the hormone's effect on metabolism.

Why do doctors order the free T3 test?

Doctors order T3 mainly to help diagnose an overactive thyroid and to find its cause, usually alongside TSH and T4, and to monitor treatment. It is not commonly used to check for an underactive thyroid, because TSH and free T4 pick that up earlier.

Free T3 is often requested after an abnormal TSH or free T4, when T3 toxicosis is suspected, or for someone on thyroid medicine who still has symptoms despite a normal TSH. MedlinePlus notes that experts consider total T3 the more accurate measure, and Testing.com says free T3 on its own does not identify thyroid disease.

What can change your free T3 result?

Free T3 is sensitive to illness, pregnancy, biotin and several medicines, so the context around the draw matters.

  • Being seriously ill, poorly nourished or recovering from surgery lowers free T3 without a thyroid problem, a pattern called non-thyroidal illness or sick euthyroid syndrome.
  • Biotin supplements can give a falsely high or low result on the method labs use; Testing.com advises stopping biotin at least 72 hours before the draw.
  • Amiodarone, corticosteroids, propranolol, lithium and anti-thyroid drugs can lower T3, some contrast dyes can affect it, and birth control pills, oestrogens and methadone can raise it. Do not stop a medicine unless your doctor tells you to.
  • Pregnancy raises the proteins that carry T3, which changes total T3 more than free T3, and free T3 falls in the second and third trimesters, so trimester ranges apply.
  • Fasting is not needed; eat and drink normally unless told otherwise.

How do you read free T3 on your lab report?

Free T3 usually sits between TSH and free T4 on a thyroid profile, printed as FT3, Free T3 or free triiodothyronine, in pg/mL or pmol/L, with the lab's range beside it. Testing.com prints 2.3–4.2 pg/mL, roughly 3.1–6.8 pmol/L, and MedlinePlus prints 210–440 pg/dL, which is the same hormone in a different unit.

Read it with the TSH and free T4 lines: Lab Tests Online UK says free T3 is always interpreted alongside both, and that it can rise before free T4 when the thyroid is overactive. A low free T3 during an illness often reflects the illness rather than the thyroid.

Amara reads free T3 with the range printed beside it on each report, flags a value outside that range, and shows what changed since your last report. Try it with two sample reports

Questions people ask about free T3

What is the difference between free T3 and total T3?

Free T3 is the small unbound part of triiodothyronine that can enter tissues, while total T3 counts that free part together with the much larger amount bound to proteins. MedlinePlus says experts consider total T3 the more accurate measurement, and Testing.com notes that pregnancy, liver disease and birth control pills change binding proteins, which moves total T3 more.

Is free T3 part of a thyroid profile?

Free T3 appears on many thyroid profiles alongside TSH and free T4, though a basic order may include only TSH. Lab Tests Online UK notes that free T3 is mainly used when an overactive thyroid is suspected and is not a routine test for people in hospital, because illness lowers it, so some panels leave it out.

Can illness lower free T3 without a thyroid problem?

Illness can lower free T3 without any thyroid disease. Lab Tests Online UK calls this non-thyroidal illness or sick euthyroid syndrome, and Testing.com adds that malnutrition and recovery from surgery do the same, as do medicines such as corticosteroids and amiodarone. A doctor reads a low free T3 with TSH, free T4 and how unwell you were.

What can a high free T3 result mean on my report?

A high flag on free T3 means the value sits above the range that lab printed. The sources say doctors most often read a high free T3, together with a low TSH, as a sign of an overactive thyroid, and biotin supplements can produce a falsely high reading. Your doctor reads it with TSH and free T4.
