Free T4 blood test: normal range explained

Also printed as: FT4, Free Thyroxine, T4 Free

Free T4 is the part of thyroxine, the main thyroid hormone, that is not attached to proteins and can enter your tissues. The adult range MedlinePlus prints is 0.8–1.9 ng/dL, or 10.3–24.5 pmol/L; Indian labs often print a narrower range, but the range printed on your own report is the one that applies.

What does free T4 measure?

Free T4 measures thyroxine (T4), the main hormone your thyroid gland releases, in the form that is not bound to carrier proteins. Most T4 in blood is bound to proteins and stays in the bloodstream as a backup supply. The small free share is the part that can enter body tissues and act. MedlinePlus says the free form is considered more accurate, so it is used more often than total T4.

Labs usually print free T4 in ng/dL, and some print pmol/L. Because the result reflects the hormone that is available to your cells, NIDDK says changes in binding proteins do not affect it the way they affect total T4.

Why do doctors order the free T4 test?

Doctors order free T4 to help find and follow an underactive thyroid (hypothyroidism) or an overactive thyroid (hyperthyroidism). It is also used to look into thyroid nodules, a goitre and problems of the pituitary gland, and to check people taking thyroid hormone medicine.

T4 alone is not enough to make a diagnosis, so it is usually ordered with TSH. NIDDK describes TSH as the first check: if TSH is not normal, at least one other test, often free T4, is needed to find the cause.

What can change your free T4 result?

Several things can move a free T4 result without your thyroid changing, so your doctor reads it with TSH and your history.

  • Pregnancy and oestrogen, including birth control pills, change the proteins that carry T4, which affects total T4 more than free T4.
  • Biotin (vitamin B7) supplements can interfere with some thyroid assays; your doctor will say whether to pause any medicine or supplement, so do not stop one on your own.
  • Severe illness, major surgery and prolonged fasting can lower T4 even when the thyroid is healthy.
  • Steroid medicines (corticosteroids) can lower T4, and too much iodine from medicines or seaweed can raise it.
  • Thyroid hormone tablets taken shortly before the blood draw can raise the reading, so Testing.com says to have blood drawn before that day's tablet.
  • Liver or kidney problems, and inherited changes in the protein that binds T4, can also affect the result.

How do you read free T4 on your lab report?

On a thyroid profile, free T4 usually sits beside TSH and free T3, printed as FT4 or Free Thyroxine with a range in ng/dL or pmol/L. Compare your value with the range on that line of your own report, because labs use different methods and print different limits.

When the test was total T4, some labs print a free T4 index (FTI), a calculated estimate, rather than a measured free T4. A value just outside the printed range does not always mean a condition needing treatment; your doctor reads it together with TSH, your symptoms and earlier reports.

Amara reads free T4 with the range printed beside it on each report, flags a value outside that range, and shows what changed since your last report. Try it with two sample reports

Questions people ask about free T4

Why is free T4 read with TSH?

Free T4 is read with TSH because the two normally move in opposite directions: MedlinePlus says that when T4 falls, the pituitary makes more TSH to push the thyroid harder. MedlinePlus says T4 alone is not enough for a diagnosis, and NIDDK describes TSH as the first check, with another test added when TSH is not normal.

What is the difference between free T4 and total T4?

Free T4 measures only the thyroxine that is not bound to proteins, the share that can enter your tissues. Total T4 counts bound and free hormone together, so it rises or falls when carrier proteins change, as in pregnancy or with oestrogen. Many doctors prefer free T4 for that reason.

Do I need to fast for a free T4 test?

Fasting is usually not needed for a free T4 test on its own. MedlinePlus says you may need to fast for several hours only when other tests on the same sample need it, and Testing.com says to eat and drink normally; follow the instructions your lab gives you for the whole form.

What does a low free T4 on my report mean?

Low free T4 means the value sits below the range your lab printed. MedlinePlus lists an underactive thyroid, some stages of thyroiditis, iodine imbalance and, less often, a pituitary problem among the reasons, along with severe illness and some medicines. Your doctor reads the result with TSH, your symptoms and your history.

Can pregnancy change a free T4 result?

Pregnancy can change thyroid test results because oestrogen raises the proteins that carry T4, which pushes total T4 up; MedlinePlus says pregnancy and oestrogen level can affect a free T4 result too. NIDDK notes that thyroid hormone levels are higher during pregnancy. Tell your doctor if you are pregnant, so the result is read against the right range.
