ALP (alkaline phosphatase) test: normal range

Also printed as: Alkaline Phosphatase, Alk Phos, ALKP

ALP, or alkaline phosphatase, is an enzyme found in all body tissues and especially in the liver, bile ducts and bone, and the blood test measures how much is in your blood. The adult range commonly printed on Indian reports is 44–147 IU/L, as MedlinePlus prints, but the range printed on your own report is the one that applies.

What does ALP measure?

ALP is an enzyme, a protein that speeds up chemical reactions. It is found in every tissue, with the highest amounts in the liver, bile ducts and bone, and each part of the body makes a slightly different form. The blood test measures the total, so on its own it cannot say which tissue the ALP came from.

When the source matters, a doctor can order an ALP isoenzyme test, which separates liver, bone, intestinal and placental ALP, or a GGT test, which rises with liver ALP but stays normal when bone is the source.

Why do doctors order the ALP test?

Doctors use ALP to screen for or help diagnose liver and bone disease. It is part of the comprehensive metabolic panel and most liver function tests, so it is often flagged on routine bloodwork; it is also ordered for symptoms such as jaundice, upper right belly pain, itching, unexplained bone pain or fractures.

A high ALP is usually followed by other tests, such as GGT, bilirubin and the liver enzymes, and sometimes imaging, before any conclusion is drawn.

What can change your ALP result?

ALP changes with age, pregnancy, meals and medicines, not only with disease.

  • Children and teenagers have naturally higher ALP because their bones are growing; during growth spurts it can reach about 500 IU/L, so adult ranges do not apply to them.
  • ALP rises in the second and third trimesters of pregnancy because the placenta makes its own form; this is expected, not a sign of disease.
  • A fatty meal before the test can raise ALP a little, particularly in people with blood group O or B, so fasting is often requested.
  • Statins, some anti-seizure medicines and some antibiotics can raise ALP, and some other medicines lower it. Do not stop a medicine unless your doctor tells you to.
  • Low zinc or magnesium can lower ALP, because the enzyme depends on zinc.
  • Healing fractures, bone disorders, blocked bile ducts and liver disease are the groups of causes doctors consider for a high value.

How do you read ALP on your lab report?

ALP is printed as alkaline phosphatase, ALP, Alk Phos or ALKP on a liver function test, usually below the transaminases, in U/L or IU/L; Testing.com notes the two units are the same.

Printed ranges vary by age, sex and lab: MedlinePlus prints 44–147 IU/L for total ALP in adults, while a children's or pregnancy range runs higher. Read your value against the range beside it, and look at GGT and bilirubin on the same report, because together they suggest whether a rise is from the liver or bone.

Amara reads ALP with the range printed beside it on each report, flags a value outside that range, and shows what changed since your last report. Try it with two sample reports

A sample Amara row for ALP: 155 U/L on 28 Jan 2026 at Lab A (sample), then 112 U/L on 16 Aug 2026 at Lab B (sample), each with its lab's printed range.

Questions people ask about ALP

Why can ALP be higher in children and teenagers?

ALP is higher in children and teenagers because growing bone produces large amounts of the enzyme. MedlinePlus says levels during growth spurts can be as high as 500 IU/L, and Testing.com says they fall to adult levels in the late teens or early twenties, so a child's result is read against a child's range, not an adult one.

Why is GGT tested together with ALP?

GGT is tested with ALP to work out where a high ALP is coming from. Both enzymes rise with liver and bile duct problems, but only ALP rises with bone disease. So a high ALP with a high GGT points doctors towards the liver, while a high ALP with a normal GGT makes bone the more likely source.

Do I need to fast before an ALP test?

Fasting is often requested for ALP. MedlinePlus says a fatty meal before the test can cause a small rise, and its isoenzyme page names people with blood type O or B as those affected. ALP is also usually ordered with other tests that need several hours of fasting, so follow the instructions on your form.

What can a high ALP result mean on my report?

A high flag on ALP means the value sits above the range that lab printed. MedlinePlus groups the reasons a doctor considers into liver and bile duct problems and bone disorders, and Testing.com adds pregnancy, adolescent growth and a fatty meal as non-disease causes. Your doctor reads it with GGT, bilirubin and your history, and may repeat it.
